Job summary

Spirit Hospice of Georgia is seeking a licensed Social Worker to join our compassionate team and support patients and families through what matters most with grace, humor, and dignity.

You will walk with patients and families through meaningful moments, coordinate care, advocate for needs, and collaborate with nurses, chaplains, aides, doctors and volunteers to ensure heart-centered care.

Qualifications

• A current Social Work License in the state of Georgia.
• Hospice, medical, or mental health experience.
• A steady heart that can hold both tears and laughter in the same breath.
• Strong communication, organizational, and advocacy skills.

• Help families navigate the messy parts.. grief, fear, and paperwork.
• Lead with compassion, advocate fiercely, and remind people that even in loss there’s still love and dignity.
• Collaborate closely with nurses, chaplains, aides, doctors and volunteers in our interdisciplinary team (IDT), ensuring care is heart-centered.
• Develop and execute patient-specific care plans
• Offer resources, counseling, and that grounding presence people remember long after the goodbye.

Spirit Hospice believes that a big heart is a superpower, and we’re looking for an additional licensed Social Worker superhero! As an equal pillar to nurses and chaplains, we want someone who can hold space for others with grace, strength, and maybe a little humor on the side.

What you bring:
