Job summary

LilyCare Hospice in New Jersey is seeking a part-time Hospice Social Worker to join our interdisciplinary team and help shape end-of-life care from the ground up. You will provide counseling and support to patients, families and caregivers, and coordinate psychosocial aspects of care.

The role is based out of Florham Park, NJ with a Monday–Friday daytime schedule, and there is an opportunity to grow into full-time work as part of our continued mission to serve the community.

Qualifications

  • MSW from a CSWE-accredited program.
  • Current NJ LSW or LCSW license issued by the State Board of Social Work Examiners.
  • One year of social work experience in a healthcare setting; hospice, home health, or geriatric experience preferred.

Responsibilities

  • Serve as the Hospice Social Worker for patients and families on your caseload; assess psychosocial needs and build the social work portion of the individualized plan of care.
  • Provide counseling and support to patients, families, and caregivers, including anticipatory grief and bereavement follow-up.
  • Evaluate insurance coverage and financial need, and connect families to community resources and benefits programs.
  • Participate in IDG meetings and keep the interdisciplinary team current on psychosocial changes.
  • Complete timely, accurate EMR documentation and support volunteer and staff orientation.

Hospice Social Worker

LilyCare Hospice is a community-based hospice serving patients and families across New Jersey. We are hiring a part-time Hospice Social Worker to join our interdisciplinary team and help shape end-of-life care from the ground up. We are looking for someone to join us part-time, with an opportunity to work full-time as well.

Duties and Responsibilities

Compensation and Schedule

The Hospice Social Worker role pays $36 - $42/hour based on experience. Full-time, Monday-Friday day schedule. Territory is northern New Jersey, based out of Florham Park, NJ.

Skills and Qualifications
  • Master of Social Work (MSW) from a program accredited by the Council on Social Work Education
  • Current New Jersey LSW or LCSW license issued by the State Board of Social Work Examiners
  • One year of social work experience in a healthcare setting; hospice, home health, or geriatric experience preferred
  • Valid driver's license and reliable transportation
