• Driver's License per Illinois SOS.
• Proof of Auto Insurance as required.
• Education: Master’s Degree: Social Work (Required)

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop bereavement plan of care for families and caregivers.
• Identify cultural, faith-based considerations and communicate implications to the care team.
• Assist with transportation, food, and utility barriers for patients at home.
• Make referrals for Medicaid, Community Care, SSDI and elder abuse/neglect as needed.
