Qualifications

• Licensed Clinical Social Worker (LCSW) licensure required

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop bereavement plans of care addressing family needs.
• Educate patients and families on cultural beliefs and spiritual considerations relevant to care.
• Document assessments and interventions in the medical record per policy.
• Collaborate with IDT to implement social work plan and coordinate community resources.
