Job summary

Heart to Heart Hospice in Indiana is seeking a dedicated Social Worker to assess and support patients with life-limiting illnesses and their families.

Applicants must hold a state-approved social work degree, an active license, and a valid driver's license with reliable transportation. Hospice experience is preferred. This is a full-time role with competitive pay and benefits, including PTO, holidays, and a 401k. A company car is available for qualifying individuals.

Qualifications

• Bachelor's or Master’s degree in social work from a State accredited program.
• Current State license required in accordance with State regulations.
• Current Driver's License and reliable transportation.
• One year of experience as a Social Worker in a clinical care setting; hospice experience preferred.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs.
• Provide individual patient/family counseling, crisis intervention, information and assistance with advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
