Care Initiatives Hospice seeks a compassionate Social Worker to deliver holistic end-of-life care across home, long-term care, assisted living, and hospital settings. You will assess psychosocial needs and collaborate with the care team to support patients and families with dignity and respect.

The role requires an MSW with LMSW license; hospice experience preferred and willingness to travel between locations within your region.

What You'll Do & Key Responsibilities:

• Assess patients’ psychosocial needs and complete social histories
• Document care plans, progress, and follow-up per regulations
• Provide counseling and emotional support to residents and families
• Support patients and families with adjustment, rights, and orientation
• Coordinate referrals and act as a liaison with community resources
• Partner with the care team to enhance quality of life and outcomes
• Plan and support patient, family, and community engagement activitiesl
• Promote facility programs and services to referral partners

What it takes to be a Social Worker at Care Initiatives Hospice:

• Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education and current, valid social work license (LMSW).
• Hospice or palliative care experience, preferred.
• Two (2) years of verifiable experience working as a Social Worker, preferred.
• Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
