Job summary

Care Initiatives Hospice is seeking a compassionate Social Worker to provide comprehensive hospice care in Cedar Rapids and surrounding areas.

You will meet patients and families where they are comfortable, offering counseling and support as part of a holistic approach addressing physical, emotional, and spiritual needs.

Qualifications

• Bachelor's Degree in Social Work is required.
• Hospice or palliative care experience is preferred.
• Two years of verifiable experience as a Social Worker is preferred.
• Valid driver's license, automobile liability insurance, and ability to travel between multiple locations within the region.

Responsibilities

• Assess patients’ psychosocial needs and complete social histories.
• Document care plans, progress, and follow-up per regulations.
• Provide counseling and emotional support to residents and families.
• Support patients and families with adjustment, rights, and orientation.
• Coordinate referrals and liaison with community resources.
• Partner with the care team to enhance quality of life and outcomes.
• Plan and support patient, family, and community engagement activities.
• Promote facility programs and services to referral partners.
