Job summary

Luminos Hospice in Texas seeks a Hospice Social Worker to provide psychosocial assessments and counseling to patients and families under the attending physician. You will collaborate with the Interdisciplinary Group/Team to tailor holistic support for individual needs.

The role requires a Master’s in Social Work, current state licensure (LCSW/LMSW or equivalent), at least two years in healthcare, and strong interpersonal and communication skills.

Qualifications

• Master’s degree in social work from an accredited program.
• Current state licensure as a social worker (LCSW, LMSW, or equivalent) required.
• Minimum two (2) years of experience in a healthcare setting, preferably hospice or palliative care.

Responsibilities

• Conduct psychosocial assessments to evaluate emotional, social, and environmental factors impacting the patient and family.
• Provide emotional support and counseling to patients and families coping with end-of-life issues.
• Coordinate care with the Interdisciplinary Group/Team to ensure all aspects of patient care are addressed.
• Assist families in understanding and managing grief and loss.
• Help families access community resources and services.
• Maintain documentation of social work services in accordance with hospice policies and regulatory requirements.

Requirements

Education:
