Job summary

ViaQuest Hospice is seeking a Medical Social Worker to join our team in Monongahela, PA. You will perform comprehensive patient assessments, conduct regular patient visits, and provide crisis intervention and counseling as needed to support patients and families.

The role requires an MSW with an active license and experience in medical social work; travel within the service area is expected. A strong emphasis on advocacy and accurate Brightree EMR documentation is essential.

Qualifications

• Master of Social Work required.
• Licensure as Social Worker in the applicable state preferred.
• Hospice social work experience preferred.

Responsibilities

• Perform comprehensive patient assessments.
• Provide patient visits as scheduled.
• Provide short term crisis intervention and individual or family counseling as needed.
• Utilize Brightree EMR for documentation.
• Serve as patient and family advocate.
