Job summary

Optimal Care seeks a Hospice Medical Social Worker to provide compassionate psychosocial care for patients and families facing end-of-life illness. You will conduct assessments, offer crisis counseling, and support advanced care planning as part of an interdisciplinary team.

The role requires LMSW/LCSW licensure, 1+ year experience, willingness to travel daily within Kent, Oceana, and surrounding areas, and reliable transportation.

Qualifications

• Master's or doctoral degree from a CSWE-accredited school of social work.
• Current LMSW or LCSW licensure in the state.
• Minimum 1 year of experience as a licensed MSW.
• Willingness to travel to homes, facilities, and community settings.
• Reliable transportation, valid driver’s license, and valid automobile insurance.

Responsibilities

• Conduct comprehensive psychosocial assessments.
• Provide crisis intervention and counseling to patients and families.
• Support advance care planning conversations and informed decisions.
• Collaborate with nurses, physicians, and hospice team to integrate psychosocial needs into the plan of care.
