Job summary

Optimal Care is seeking a licensed Hospice Medical Social Worker to provide compassionate psychosocial care for patients and families facing end‑of‑life illness. You will conduct psychosocial assessments, offer crisis counseling, and support advance care planning.

This role involves daily travel within the service area to patients’ homes and facilities. You’ll collaborate with nurses, physicians, and other hospice staff to ensure culturally sensitive, holistic care during the hospice journey.

Qualifications

• Master’s or doctoral degree from a CSWE-accredited school of social work
• Current state licensure as LMSW or LCSW
• Minimum 1 year of licensed MSW experience
• Reliable transportation, valid driver’s license, and automobile insurance
• Willingness to travel to homes, facilities, and community settings
