Job summary

Qualifications

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop bereavement plan of care addressing family/caregiver needs.
• Interpret and communicate patient/family faith and cultural beliefs to the care team.
• Educate patients and families and overcome barriers; tailor to learning abilities.
• Document assessments and interventions in the electronic medical record and ensure timely updates.
