Job summary

Hospice Foundation Of Greater Baton Rouge is seeking a licensed Master’s in Social Work to join our team, providing compassionate psychosocial support to patients and families in home and facility settings.

Responsibilities include developing care plans, counseling patients and families, and collaborating with interdisciplinary teams to address psychosocial needs. Travel to patient homes is required; own transportation and proof of liability insurance meeting state requirements are needed.

Qualifications

• Master’s degree in Social Work is required.
• Licensure as LMSW or LCSW in Louisiana is required or pending.
• Previous hospice experience or strong understanding of hospice care concepts preferred.

Responsibilities

• Maintain a positive attitude and uphold the organization’s mission.
• Maintain professionalism in all interactions with staff, patients and families.
• Develop and revise patient/family care plans and participate in the care planning process.
• Assess psychosocial needs and coping skills related to terminal illness.
• Provide counseling services for patients and families.
