The role requires a Master’s in Social Work, current Texas licensure, reliable transportation, and strong communication. You’ll collaborate across care settings, document care, and participate in on-call rotations.

Qualifications

• Master's Degree in Social Work required.

• Current Texas social work licensure required.
• Reliable transportation; Texas driver’s license and auto insurance.

• Prior work experience with demonstrated competency in social work duties.
• Demonstrates integrity, professional presence, and strong work ethic.
• Strong written and verbal communication; able to document clearly.

Responsibilities

• Plan of Care and Direct Services for psychosocial needs and discharge planning.
• Coordinate with the interdisciplinary team to support patient and family needs.
• Provide bereavement services and crisis intervention as required.
• Assist with admissions and family meetings; liaison with community resources.
