The ideal candidate holds a Master’s in Social Work and has experience in healthcare hospice settings, with strong communication and collaboration skills to work with an interdisciplinary team.

Qualifications

• Master’s degree in Social Work from a university accredited by the Council on Social Work Education.
• One year of social work experience in a health care setting; hospice experience preferred.

Responsibilities

• Conducts biopsychosocial assessments of the patient and family as the basis for care planning.
• Develops and maintains therapeutic relationships.
• Addresses emotional, spiritual, and psychological needs of the patient and family related to illness and care.
