Job summary

Gilchrist Inova is seeking a licensed master social worker to join the interdisciplinary hospice team serving Northern Virginia. You will assess patients and families, develop and implement care plans, and provide counseling about dying, coping, and planning.

You will connect families with resources and support throughout the care journey. The role requires LMSW/LCSW/LCSW-C with licensure advanced within 3 years, strong communication, time management, and cultural humility, plus ability to drive

Qualifications

• Master’s degree in Social Work from CSWE-accredited program.
• LMSW, LCSW, or LCSW-C licensure in Virginia (advanced LMSW within 3 years).
• Proficiency in conducting biopsychosocial assessments and care planning.

Responsibilities

• Conduct biopsychosocial assessments of patient and family for care planning.
• Develop and maintain therapeutic relationships and provide counseling.
• Address emotional, spiritual, and psychological needs related to illness and end-of-life care.
• Provide caregiver education and connect to community resources.
• Participate in IDT meetings, discharge planning, and after-hours coordination.
