Job summary

Compassus in Michigan is seeking a Hospice Social Worker to provide psychosocial assessment, counseling, and support to patients and families within the interdisciplinary team. You will help patients maximize coping and coordinate care with community resources.

You will support care planning, advance directives, and linkage to resources while upholding respect for diverse backgrounds and nondiscriminatory practice. Requires MSW and state licensure where applicable.

Qualifications

• MSW from CSWE-accredited program with healthcare experience.
• Bachelor’s degree in social work or related field with supervised MSW option.
• State social work licensure required where you practice.

Responsibilities

• Explain hospice services and Medicare benefits to patients and families.
• Submit required paperwork and ensure timely documentation.
• Participate as a member of the IDT and contribute to care plans.
• Provide psycho-social support and counseling to patients and families.
• Coordinate with IDT to ensure continuity and quality of care.
• Maintain updated care plans reflecting problems, goals, and interventions.
• Facilitate advance planning including living will/POA and funeral arrangements.
• Assist in long-term care placement and monitor adjustments.
