Job summary

Abode Hospice of Colorado seeks a compassionate Social Worker to join our Pueblo team. You will assess psychosocial needs, provide counseling and crisis support, and coordinate resources for patients facing terminal illness.

Responsibilities include maintaining documentation, educating families on advance directives, and collaborating with a multidisciplinary team to ensure holistic, dignified care in the home setting. Schedule is Mon-Fri, 8:00 AM - 5:00 PM.

Qualifications

• Master's degree in social work (MSW) or Bachelor's in social work accredited by CSWE.
• Minimum one year of health care experience; hospice experience preferred.
• Strong verbal and written communication and organizational skills.
• Current licensure to practice in the state of operation and a valid driver’s license.

Responsibilities

• Assess psychosocial needs of patients and families.
• Provide counseling, crisis intervention, and emotional support.
• Maintain accurate and timely clinical documentation.
• Educate and assist with advance directives.
• Collaborate with the care team to support holistic patient care.

Education

MSW accredited by CSWE (or Bachelor's in Social Work CSWE)
