Haven Hospice in DeLand, FL is seeking a compassionate Social Worker to support patients and families facing terminal illness. The role focuses on psychosocial assessment, counseling, and coordinating resources to ensure dignified, patient-centered care.

The position offers a Monday–Friday schedule (8:00–5:00) with opportunities for professional growth, teamwork, and meaningful impact in the community.

Qualifications

• Master's in Social Work (MSW) accredited by CSWE.
• Bachelor's in Social Work accredited by CSWE.
• Minimum of one year healthcare experience; hospice experience preferred.

Responsibilities

• Assess psychosocial needs of patients and families.
• Provide counseling, crisis intervention, and emotional support.
• Maintain accurate and timely clinical documentation.
• Educate and assist with advance directives; provide referrals and practical support.
