Qualifications

• MSW from an accredited school/university.

• Knowledge of human development, end-of-life care, and family systems.
• Able to remain calm and professional and troubleshoot crises.

Responsibilities

• Conducts initial and ongoing psychosocial, spiritual, and pre-bereavement assessment of patient/families.
• Develops and implements patient/family personalized care plans with counseling and referrals.
• Provides case management for psychosocial needs of patients/families.
• Completes clinical documentation and bereavement assessments in a timely manner.
• Participates in interdisciplinary team meetings and agency tasks.
• Maintains flexible hours including evenings/weekends as needed.
• Promotes confidentiality and ethical practice; adheres to policies.
• Supports community partnerships and professional involvement.
