Job summary

Qualifications

• Bachelor's degree in Social Work or a similar major.
• One year of social work experience in a health care setting.
• Current CPR certification required.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Visits patients according to Plan of Care and submits clinical notes daily.
• Act as Bereavement Coordinator and supervise bereavement services for up to 13 months after death.
• Assist with Social Work and Bereavement policy development and revisions.
