Job summary

Corewell Health in Grand Rapids, MI seeks a Social Worker to join the Hospice West team. You will perform psychosocial assessments, coordinate with alternate sites of care, and document plans in the EMR. The role includes providing resources to the care team and participating in committees.

Required MSW with MI licensure on hire; MI driver's license for hospice/home care on hire. Full-time, day shift, 40 hours/week with on-call weekends, within a supportive, patient-centered program.

Qualifications

• Master's degree in Social Work is required.
• 2 years of health care experience post-MSW preferred.
• LIC-Master Social Worker (MI) upon hire; LIC-Driver's License for MI Hospice/Home Care upon hire.

Responsibilities

• Conduct psychosocial assessments with patients/families to identify needs and plan.
• Coordinate with alternate sites of care and community agencies.
• Document assessments and plans in the EMR, reflecting current condition and needs.
• Provide consultation and resources to the healthcare team.
• Maintain knowledge of community resources and regulatory requirements.
• Participate in department, regional, or statewide committees as assigned.
• Provide short-term therapeutic support when appropriate.
