Job summary

Moments Hospice is seeking a licensed social worker to provide comprehensive psychosocial support to terminally ill patients and their families. The role emphasizes compassionate care and collaboration with leadership to uphold hospice principles.

You will work with an interdisciplinary team, manage caseloads, and participate in weekly meetings. A driver’s license is required, and a company car is provided for business and personal use with minimal cost.

Qualifications

• Master’s degree in Social Work preferred; BSW with healthcare experience acceptable.
• At least 2 years of social work in health care or social services.
• Current Social Worker license required.
• Hospice experience a plus.
• Valid driver’s license and auto insurance required.

Responsibilities

• Provide psychosocial services to terminally ill patients and families.
• Contact patients/families within 48 hours of admission.
• Case Manager duties.
• Psychosocial assessment within 24 hours of completion.
• Regularly assess psychosocial and bereavement needs.
• Participate in weekly interdisciplinary team meetings.
• Assist with funeral arrangements if requested.
• Provide counseling during illness and at time of death.
