Job summary

Moments Hospice in Wisconsin is seeking a licensed social worker to join our on-the-road team, providing psychosocial support to terminally ill patients and their families. The role emphasizes autonomy, collaboration with leadership, and compassionate care.

Qualifications

MSW preferred; BA in Social Work with healthcare experience acceptable.
Minimum 2 years social work experience in healthcare or social services.
Current Social Worker license required.
Hospice experience a plus.
Knowledge of community resources and eligibility programs.
Understand hospice philosophy and death/dying.
Strong organizational and communication skills.
Valid drivers license and valid auto insurance required.

Responsibilities

Provide a wide range of psychosocial services to terminally ill patients and families.
Contact patient, family, and caregiver within 48 hours of admission.
Perform case management duties.
Complete psychosocial assessment within 24 hours of completion.
Regularly assess psychosocial needs of patient and family.
Assess pre-bereavement/anticipatory grief needs and implement Plan of Care.
Re-evaluate needs; participate in weekly interdisciplinary team meetings.
Assist with funeral arrangements if requested.
Provide counseling during illness and at time of death if requested.
