Job summary

Moments Hospice is seeking a licensed Social Worker to provide psychosocial support for terminally ill patients and families. This on‑the‑road position offers autonomy, a company car, and a comprehensive benefits package.

You will engage with patients within 48 hours of admission, perform psychosocial assessments within 24 hours, and participate in interdisciplinary team meetings. Required MSW (or BA with healthcare experience) and an active license; hospice experience is a plus.

Qualifications

• Current Social Worker license is required.
• Hospice experience is a plus.

Responsibilities

• Provide wide range of psychosocial services to terminally ill patients and their families.
• Make contact with patient, family, and caregiver within 48 hours of admission.
• Case Manager duties.
• Psychosocial assessment must be part of patient's chart within 24 hours of completion.
• Regularly assess the patient and family psychosocial needs.
• Assess pre-bereavement/anticipatory grief needs and implement the Plan of Care.
• Regularly re-evaluate the patient's and family's psychosocial needs.
• Participate in weekly interdisciplinary team meetings.
• Assist with funeral arrangements if requested.
• Provide counseling during the process of the terminal illness and at the time of death if requested
