Job summary

Three Oaks Hospice in Pennsylvania seeks a licensed social worker to deliver in-home hospice support and coordinate psychosocial care for patients of all ages.

You will perform assessments, counseling, and referrals, collaborating with IDG/IDT to ensure seamless care while upholding privacy and dignity for patients and families.

Qualifications

Bachelors or masters degree in social work from a State accredited program If has a bachelors degree and is hired after 12022008 must be supervised by a Master of Social WorkCurrent and valid license as a LMSWWorking knowledge of community resources Good interpersonal skills Hepatitis profileCurrent State CertificationReliable transportation Valid Drivers License and current auto liability insurance
