Responsibilities

The Medical Social Worker for hospice provides medical social services under the direction of a physician and Interdisciplinary Group to assist in understanding significant social and emotional factors related to the patient's health status and in developing coping mechanisms. Responsibilities include:

• Assisting patients, significant others, physicians, and healthcare team members in understanding personal, emotional, environmental, and social factors affecting health.
• Contributing to the development of a comprehensive, integrated Plan of Care for patients daily.
• Instructing healthcare team members on community resources available to assist patients.
• Functioning as a Bereavement Coordinator and supervising bereavement services as needed.
• Establishing a Plan of Care addressing bereavement needs, services, and frequency up to thirteen months after the patient's death.

Education and Experience

License Requirements

• Master's Degree in Social Work from an accredited school.
• One year of social work experience in a healthcare setting.
• Current CPR certification.
• Valid Driver's License, vehicle insurance, and access to reliable transportation.

Additional State Specific Requirements

• MN: Must be licensed as a Social Worker from the State Board of Social Work.
