Job summary

Elevate Hospice is seeking a compassionate Social Worker to support patients and families facing terminal diagnoses in Leawood, KS. You will assess psychosocial needs, provide counseling, guide advance care planning, and connect families with community resources to ease practical and emotional burdens.

The role emphasizes collaboration with an interdisciplinary team, documenting interventions, and participating in on-call coverage.

Qualifications

  • BSW required; MSW or LCSW preferred.
  • At least 1 year in social work, preferably hospice/home health.
  • Strong written and verbal communication skills.
  • Current BLS/CPR certification.
  • Valid driver’s license and reliable vehicle.

Responsibilities

  • Conduct psychosocial assessments of patients and caregivers.
  • Provide counseling on emotional stress, coping, grief, and advance care planning.
  • Assist with advance directives and navigating legal/financial decisions.
  • Identify and coordinate with community resources and support organizations.
  • Educate staff and families about cultural and psychosocial needs.
  • Document patient conditions and psychosocial interventions.
  • Liaise between families and care teams for coordinated care.
  • Participate in interdisciplinary team meetings to develop care plans.
  • Provide crisis intervention and safety support.
  • Participate in quality improvement initiatives and continuing education.
  • Participate in on-call support as needed.

At Elevate Hospice, we’re committed to caring for patients and families with compassion and dignity. We are seeking a skilled and empathetic Social Worker (SW) to join our team. In this vital role, you'll help patients and families cope with the emotional, social, and practical challenges that come with a terminal diagnosis—bringing comfort and clarity during their most vulnerable moments.

What You'll Do
  • Conduct psychosocial assessments of patients and caregivers
  • Provide counseling to patients and families regarding emotional stress, coping, grief, and advance care planning
  • Assist with advance directives, funeral arrangements, and navigating legal/financial decisions
  • Identify and coordinate with community resources, agencies, and support organizations
  • Educate staff and families about cultural and psychosocial needs
  • Document patient conditions, interventions, and psychosocial support provided
  • Serve as a liaison between families and care teams to ensure coordinated, holistic care
  • Participate in interdisciplinary team meetings to develop individualized care plans
  • Provide crisis intervention and support for safety concerns (neglect, abuse, self-harm)
  • Participate in quality improvement initiatives and continuing education
  • Participate in on-call support as needed
Qualifications
  • Bachelor’s degree in Social Work (BSW) required
  • Master’s degree (MSW) or LCSW strongly preferred
  • At least 1 year of experience in social work, preferably in hospice, home health, or a community setting
  • Knowledge of end-of-life care, grief support, and cultural sensitivity
  • Strong written and verbal communication skills
  • Self-motivated, organized, and able to work independently
  • Current Basic Life Support (CPR) certification
  • Valid driver’s license, auto insurance, and reliable vehicle required
