Serenity Hospice in Gettysburg, PA is hiring a Hospice Social Worker for a full-time role. The position focuses on providing emotional support, crisis intervention, and resource coordination for patients and families navigating end-of-life care.

You will work with an interdisciplinary team to develop care plans, access community resources, and support advance directives. This role emphasizes empathy, dignity, and compassionate guidance in every interaction.

Qualifications

• Bachelor’s or Master’s degree in Social Work (BSW/MSW).
• Valid Pennsylvania Social Work license.
• Pennsylvania residency.
• Hospice or healthcare experience preferred.
• Strong communication and interpersonal skills.
• Empathy, resilience, and professionalism.

Responsibilities

• Conduct psychosocial assessments and provide ongoing emotional support.
• Develop individualized care plans alongside the interdisciplinary team.
• Help patients and families access community resources and supportive services.
• Assist with advance directives and end-of-life planning.
• Provide crisis intervention and compassionate guidance.
• Offer bereavement support to families.
• Maintain accurate, timely, and professional documentation.
