Qualifications

• Master's in Social Work from a CSWE-accredited program.
• Active Arizona LMSW or LCSW license in good standing.
• Minimum of three (3) years of professional social work in hospice, palliative care, healthcare, home health, or related clinical setting.
• Minimum of one (1) year of leadership, supervisory, or management experience.
• Valid Arizona driver's license and reliable transportation.

Responsibilities

• Supervise, mentor, and support a team of Licensed Social Workers.
• Oversee psychosocial assessments and care planning activities.
• Collaborate with nurses, physicians, chaplains, and other interdisciplinary team members.
• Assist patients and families with emotional, financial, social, and end-of-life challenges; crisis intervention as needed.
• Ensure compliance with Medicare Conditions of Participation, AZ regulations, HIPAA, and hospice standards; participate in quality initiatives.
