Job summary

Summit Home Care and Hospice is seeking a registered nurse to join our hospice team. The RN will plan, direct, and evaluate care for terminally ill patients in their homes, coordinating with families and physicians to promote comfort and quality of life.

The full-time role involves on-the-road patient visits with occasional office training, plus an on-call rotation. An active RN license, CPR, and driver’s license are required; hospice experience is preferred.

Qualifications

• Graduate from an accredited school of nursing (associates or bachelor’s).
• 1 year RN experience required.
• Prior hospice experience preferred.
• CPR certification required.
• Must be a licensed driver and insured.

Responsibilities

• Provide direct hospice care to patients and families in home settings.
• Offer triage support and caregiver education.
• Administer medications and treatments as prescribed.
• Document clinical notes and update the physician as needed.
• Coordinate with the physician, order handling, and patient safety assessments.
• Participate in on-call duties and interdisciplinary team meetings.
