Job summary

AccentCare is seeking a full-time Hospice Registered Nurse Team Director in Newark, Delaware. The role combines clinical leadership with supervision of the patient care team across hospice services.

The ideal candidate has an RN license, supervisory experience in hospice or home health, and an Associate degree (CHPN preferred). Travel to all business locations is required, with on-site scheduling and program support.

Qualifications

• Associate degree preferred.
• Certified Hospice Nurse (CHPN) preferred.

Responsibilities

• Supervise direct patient and family care team members, in collaboration with program site leadership.
• Schedule, payroll, and other administrative duties.
• Ensure initial assessments and required reassessments are conducted by qualified staff within required timeframes.
• Oversee and support the Interdisciplinary Group (IDG) in care planning for each patient and family.
• Collaborate with site leadership to monitor staff performance, productivity, and documentation.
• Support Back to Bedside efforts and promote Case Management skills across disciplines.
• Facilitate staff education and IDG meetings; provide triage and caller support as needed.
• Participate in or lead quality improvement activities and data collection.
• Reinforce internal and external communications.
