Absolute Hospice, Inc. is seeking a dedicated Hospice RN for Weekend and Evening On-Call Coverage in Canton, OH. You will build trusted relationships with patients and families, provide hands‑on nursing care, and manage clinical tasks in home care settings.

The role requires a ASN in Nursing, 2+ years nursing experience (hospice/home health preferred), and a valid driver's license. Strong communication and charting skills are essential for success in this PRN, on‑call position.

Qualifications

• Associate's of Science degree in Nursing (ASN) or equivalent is required.
• Minimum 2 years of nursing experience, hospice or home health preferred.
• Valid driver’s license and clean driving record.
• Excellent communication, writing and charting skills.

Responsibilities

• Continually evaluate patient needs and adjust care plans.
• Initiate, coordinate and revise treatment plans.
• Provide hands‑on nursing care and procedures per physician orders.
• Participate in QA/QI processes and documentation assurance.
• Prepare accurate and timely clinical reports and records.
• Adhere to professional standards in patient care.

The Hospice RN establishes a professional and caring relationship with our patients and families, support staff, physicians, discharge planners, social workers, facilities and other health care professionals by providing hands-on-care, on-going clinical management and education regarding hospice care.

As a Hospice RN, you will educate patients and their caregivers on their conditions and the steps needed to ensure proper recovery.

Additional responsibilities of our Hospice RN include:

• Continually evaluating the patient's needs
• Initiating, coordinating and revising the treatment plan
• Rendering nursing care and performing treatments, as ordered by the physician
• Participating in the Quality Assurance/Quality Improvement process
• Preparing accurate, orderly and timely reports and documentation
• Adhering to all practice standards as they apply to patient care

Requirements:

• Caring, patient and flexible teacher with excellent communication and time management abilities
• Possess independent problem solving abilities, excellent clinical assessment skills
• Good physical shape
• Ability to lift/transfer patients to and from wheelchairs, if necessary
• Associate's of Science degree in Nursing
• Minimum 2 years of nursing experience including hospice or home health experience preferred
• Valid driver's license and one year clean driving record
• Excellent writing, communication and charting skills
