Job summary

Absolute Hospice, Inc. in Canton, Ohio is seeking a Registered Nurse RN - Hospice - PRN to join our family-owned agency.

The Hospice RN will provide hands-on nursing care at patients' homes, educate families, and coordinate care with physicians, social workers and discharge planners. Ideal candidates have 2+ years of nursing experience, with hospice or home health preferred, and possess strong communication, adaptability and physical stamina to lift/transfer patients when necessary.

Qualifications

• Minimum 2 years of nursing experience including hospice or home health preferred.
• Ability to lift/transfer patients as needed.
• Valid driver's license and clean driving record.
• Excellent writing, communication and charting skills.

Responsibilities

• Continually evaluating the patient’s needs.
• Initiating, coordinating and revising the treatment plan.
• Rendering nursing care and performing treatments, as ordered by the physician.
• Participating in the Quality Assurance/Quality Improvement process.
• Preparing accurate, orderly and timely reports and documentation.
• Adhering to all practice standards as they apply to patient care.

The Hospice RN establishes a professional and caring relationship with our patients and families, support staff, physicians, discharge planners, social workers, facilities and other health care professionals by providing hands-on-care, on-going clinical management and education regarding hospice care.

As a Hospice RN, you will educate patients and their caregivers on their conditions and the steps needed to ensure proper recovery.

Additional responsibilities of our Hospice RN include:

• Continually evaluating the patient’s needs

Requirements
