This role requires establishing relationships with patients, ongoing clinical management, and patient education. Candidates should have an Associate's degree in Nursing, a valid Ohio RN license, and a minimum of 2 years of nursing experience, preferably in hospice or home health. Strong communication skills and the ability to adapt are essential for success in this role.

Qualifications

The Hospice RN establishes a professional and caring relationship with our patients and families, support staff, physicians, discharge planners, social workers, facilities and other health care professionals by providing hands-on-care, on-going clinical management and education regarding hospice care.

As a Hospice RN, you will educate patients and their caregivers on their conditions and the steps needed to ensure proper recovery.

Additional responsibilities of our Hospice RN include:

• Making initial nursing evaluation visit
• Continually evaluating the patient's needs
• Initiating, coordinating and revising the treatment plan
• Rendering nursing care and performing treatments, as ordered by the physician
• Participating in the Quality Assurance/Quality Improvement process
• Preparing accurate, orderly and timely reports and documentation
• Adhering to all practice standards as they apply to patient care

Requirements

• Caring, patient and flexible teacher with excellent communication and time management abilities
• Possess independent problem solving abilities, excellent clinical assessment skills
• Computer literate
• Good physical shape
• Ability to lift/transfer patients to and from wheelchairs, if necessary
• Associate's of Science degree in Nursing
• Valid Ohio RN license
• Minimum 2 years of nursing experience including hospice or home health experience preferred
• Valid driver's license and one year clean driving record
• Excellent writing, communication and charting skills
