Job summary

Amedisys, part of the Optum family, is seeking a Per diem Hospice Registered Nurse to deliver compassionate, patient-centered home-based care. You will assess needs, coordinate individualized care plans, manage symptoms, educate families, and collaborate with the clinical team in a home-care setting.

You will perform initial assessments, develop and revise care plans, and communicate progress to the care team. On-call duties and other duties may be required as part of this role.

Qualifications

• Current RN license in applicable state(s).
• CPR certification is required.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the total plan of care with the clinical staff to maintain continuity of patient care.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Educates families on patient condition, needs and care.
• Performs on-call responsibilities and on-call services as assigned.
• Performs other duties as assigned.
