You will perform comprehensive assessments, set goals for symptom management, and collaborate with an interdisciplinary team to address patient needs holistically.

Qualifications

Registered Nurse with CT RN license.
One year of RN experience; Homecare or Hospice experience preferred.
Valid driver’s license and reliable transportation.
Computer proficiency required.

Responsibilities

Support our patients, families through end of life journey.
Perform comprehensive assessment to establish goals for symptom management and end-of-life care.
Be a member of an interdisciplinary team addressing patients' needs holistically.
Develop and maintain therapeutic relationships through effective communication.
Utilize state-of-the-art technology to document skilled services.
