Job summary

Legacy Hospice in Leawood, KS is seeking a skilled Patient Care Coordinator (RN) to lead the clinical team and ensure the highest quality of patient care in line with our mission and values.

You will oversee day-to-day coordination of patient care, supervise the interdisciplinary team, participate in hiring and training, manage admissions/discharges, and support compliance with regulations while delivering compassionate, patient-centered care.

Qualifications

• RN license in the state of employment.
• Minimum 1 year hospice, home health, or related nursing experience.
• Experience supervising clinical and paraprofessional staff.
• Active BLS (CPR) certification.

Responsibilities

• Oversee day-to-day coordination of patient care.
• Supervise the interdisciplinary clinical team.
• Support Branch Administrator in operations and compliance.
• Lead IDT meetings and ensure regulatory readiness.
• Provide direct patient care and RN case management as needed.
• Collaborate with community providers and internal departments.
