Job summary

Agrace is seeking a part-time RN in the La Crosse, WI area to provide patient- and family-centered hospice and palliative care in new markets. The RN will assess needs, admit patients to homecare, and ensure care aligns with Agrace standards and policies.

The role includes on-call responsibilities (3-4 weeknights/month, occasional weekends) and 8a-5p daytime shifts, Monday–Friday, with 24 hours per week. A valid WI RN license and reliable transportation are required.

Qualifications

• Current, unrestricted Registered Nursing License in state of employment.
• One-year nursing experience, preferred.
• Valid driver’s license and access to reliable vehicle.
• Ability to develop and follow plans of care with patient and family.

Responsibilities

• Assess patients and family needs per the plan of care.
• Provide interventions, pain management, and education.
• Develop and revise the plan of care with the IDT and family.
• Oversee CNA/LPN work and delegate visits as needed.
• Coordinate with referral sources and Agrace programs.
