Amedisys in Garner, NC is seeking a Hospice RN On-Call to provide after-hours clinical support, triage patient needs, and coordinate care across remote and field settings. You will assess conditions by phone and in person, collaborating with physicians and care teams to ensure continuity and high-quality outcomes.

Responsibilities include patient and caregiver education on symptom management, accurate documentation, and timely escalation when needed.

Qualifications

• Current RN license for the state(s) you are assigned to work (as required).
• 1+ year nursing experience, preferably in hospice, home health, or acute care.
• Current CPR certification is required.
• Valid driver’s license, reliable transportation, and liability insurance.

As the Hospice RN On‑Call, you are responsible for providing after‑hours clinical support, triaging patient needs, and coordinating care to ensure continuity and quality outcomes. This role delivers remote and field‑based nursing support, assesses patient conditions, and collaborates with interdisciplinary teams to address urgent and emergent patient needs.

Primary Responsibilities:

• Provide on‑call nursing support, triaging patient needs and determining appropriate interventions
• Assess patient conditions via phone and/or in‑person visits and elevate care as needed
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery
• Deliver patient and caregiver education related to symptom management and care plans
• Maintain accurate and timely documentation of all patient interactions and clinical decisions

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $79,600 to $119,400 annually based on full‑time employment. We comply with all minimum wage laws as applicable.
