Silverado Hospice is seeking an RN On-Call to provide after-hours nursing support for our Fort Worth area patients and families. You will perform after-hours assessments, adjust care plans, and coordinate with physicians to ensure timely, compassionate care in life-limiting situations.

Qualifications

• Must be a Registered Nurse with a valid state license.
• Minimum of 1 year of nursing experience.
• Hospice experience preferred, but not required.
• Must clear a criminal background check, physical, and drug screening.
• Ability to lift 50 pounds.
• Must have a valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive nursing evaluations and after-hours assessments for patients with changes in condition.
• Assess for signs and symptoms of discomfort, including pain, and take prompt, appropriate action.
• Initiate and update plans of care as needed; provide nursing follow-up and caregiver instruction.
• Submit accurate documentation within 24 hours of visits.
• Coordinate services across settings to ensure quality and appropriateness of care.
• Follow approved care plans, modify as needed, and communicate with physicians when appropriate.

Anticipated pay range: $38 USD - $45 USD
