You will perform initial nursing assessments, coordinate care plans, document progress, educate families, and participate in on-call services. The role offers a salary of $83,000–$87,000 with mileage reimbursement and benefits starting the first month.

Qualificações

• Current RN license valid for the state(s) assigned.
• At least 1 year of nursing experience.
• Current CPR certification.

Responsabilidades

• Respond to after-hours telephone requests or perform in-person visits with skilled assessment and interventions.
• Complete, coordinate and revise the total plan of care with continuity of patient care.
• Regularly communicate patient progress to the clinical manager, physician and care team.
• Develop and maintain individualized patient care progress records accurately and timely.
• Record pain/symptom management changes and outcomes as appropriate.
• Educate families on patient condition, needs and care.
• Make referrals to other hospice care disciplines as needed or document rationale.
• Perform on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Overview

Hospice RN On Call

Respond to telephone requests at home from our after-hours Triage RN for assistance by addressing the request by telephone or by making an in person visit that includes skilled assessment and interventions which can include in person after hours admissions and end of life visits.

• Performs initial nursing assessment on new admissions.
• Completes, coordinates and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One+ year of nursing experience required
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
