Job summary

Horizon Home Care & Hospice, Inc. is hiring a part-time RN for their Hospice Inpatient Unit in Milwaukee, Wisconsin. Qualifications

• 1-2 years of hospice, oncology or related experience.
• Experience in the care of the terminally ill.
• Hospice certification required within 18 months of employment.

Responsibilities

• Perform referral evaluations and admission assessments.
• Document patients and families’ care expectations.
• Provide case-management functions for assigned caseload.
