Job summary

VITAS Healthcare is seeking a registered nurse for full-time case management in Bakersfield, CA. The role includes supervising hospice aides and licensed nurses, conducting comprehensive assessments, coordinating care plans, and educating patients and families.

You will participate in weekly team meetings, on-call shifts, and ensure documentation at the point of care in alignment with VITAS standards. Experience in hospice/home health is preferred.

Qualifications

• California RN license required.
• Minimum of two years in hospice, home health, or community health in the last five years.
• Experience with team supervision and care coordination.

Responsibilities

• Supervises Hospice Aides, LPNs, and Volunteers per the plan of care.
• Completes initial and ongoing comprehensive assessments (physical, psychosocial, spiritual, bereavement).
• Identifies/utilizes resources and referrals (PT, OT, Home Health, Dietary, Pharmacy, supplies, equipment).
• Documents at the time of the event and at the location of care.
• Provides education to patients, families, caregivers and referral sources.
• Participates in patient care meetings and weekly team meetings.
• Provides shift reports and participates in on-call rotation as needed.
• Develops, coordinates, and follows the patient’s individualized plan of care.
• Delegates nursing tasks safely to Hospice Aides/Volunteers/LPNs.
