Qualifications

• Current Michigan License in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Ability to participate in field visits daily, weekly office meetings as indicated.
• Comfort with on-call responsibilities.

Responsibilities

• Provide one-on-one home-based hospice care.
• Supervise and support team members during visits.
• Participate in IDG conferences and care plan development.
• Educate patients and families on disease processes and end-of-life care.
• Maintain and update the patient/family plan of care.
