Three Oaks Hospice, Inc. in Willis, TX is seeking an experienced Registered Nurse to join our hospice team, delivering skilled nursing care in patients' homes and coordinating with the interdisciplinary team.

You will assess patients, implement care plans, educate families, and participate in on-call rotations, ensuring compassionate, quality end-of-life care across Conroe, Willis and surrounding areas. The ideal candidate holds an RN license, associates or bachelor’s degree, CPR, and at least

Qualifications

Current state RN license.
Two years of RN experience in a clinical care setting; hospice or home health preferred.
Current CPR certification.
Proficient with computers; Microsoft Office experience preferred.
Strong interpersonal, communication, and organizational skills.
Demonstrate ethical conduct and compliance with standards.

Responsibilities

Initial and ongoing comprehensive assessments of patient needs and ADLs.
Implement individualized POC and revisions to the plan as necessary.
Educate patient and family about disease, self-care, end-of-life care; train staff as needed.
Initiate preventive and rehabilitative nursing procedures.
Prepare progress notes showing progress toward goals.
Coordinate all patient/family services with the IDT; participate in on-call duties.
