Job summary

Hartford HealthCare at Home in Bridgeport, CT is seeking an RN to provide skilled nursing care to hospice clients in home and community settings, ensuring high-quality, cost-effective care. You will review referrals, perform comprehensive nursing assessments, develop care plans, coordinate with the IDT, and educate clients and families on symptoms, medications, and end-of-life care.

Requires graduation from an accredited nursing program, CT RN license, and at least 1 year of nursing experience,

Qualifications

Graduate of an Associate's or diploma nursing program.
Minimum of 1 year nursing experience, preferably in hospice or home care.
Current CT nursing license/permit to practice.

Responsibilities

Provide skilled nursing care to hospice clients in their homes or community health settings.
Review referrals, initiate contact with client and family.
Conduct complete nursing assessments and develop plan of care with the client and disciplines.
Coordinate with interdisciplinary team and community resources.
Educate clients and families on symptoms, medications, and end-of-life care.
