Responsibilities

• Direct Clinical Care: assess, provide wound care, IV therapy, meds, and symptom management.
• Care Planning & Coordination: develop and evaluate individualized care plans with the team.
• Patient & Family Support: educate on care management and disease progression.
• Team Supervision: supervise and delegate to LPNs/HHA and support staff.
• On-Call & Emergency Response: participate in on-call rotations and urgent responses.
• Documentation & Compliance: maintain accurate records per regulations and policies.
