Job summary
Top Echelon LLC is seeking a compassionate Registered Nurse (RN) in Scranton/Dickson City, PA to deliver patient-centered care and coordinate with an interdisciplinary team. Responsibilities include direct clinical care, care planning, patient education, and supervision of nursing staff.
The role requires PA RN licensure, 1 year of clinical RN experience (home health or hospice preferred), CPR/BLS, and a valid driver’s license for local travel within the service area.
Qualifications
- Graduate of an accredited School of Nursing (ASN or BSN).
- Minimum of one year of clinical RN experience (home health or hospice preferred).
- Current, unencumbered RN license in Pennsylvania.
- Active CPR/BLS certification.
- Valid driver’s license with reliable vehicle and willingness to travel within the service area.
Responsibilities
- Direct clinical care including wound care, IV therapy, medications, and symptom management.
- Develop and evaluate individualized care plans with physicians and the care team.
- Educate patients and families on care management and health maintenance.
- Oversee and delegate tasks to LPNs/LVNs, HHAs, and nursing staff.
- Participate in on-call rotations and urgent clinical responses.
- Maintain accurate clinical records in compliance with regulations and policies.
