Job summary

Pathways Healthcare is seeking a Registered Nurse - Hospice to join our clinician-led home health and hospice team serving Salem and surrounding areas. You will provide compassionate, patient-centered care within the patient's home environment, emphasizing dignity and quality of life.

Ideal candidates have 2+ years nursing experience, preferably in geriatrics, and hold an accredited nursing degree with current BLS CPR.

Qualifications

• 2+ years nursing experience, preferably in geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records (EMR).
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Assesses and evaluates patient’s status by writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals based on nursing diagnosis and palliative nursing actions.
• Initiates appropriate preventive and rehabilitative nursing procedures; administers medications and treatments as prescribed by the physician.
• Completes, maintains and submits accurate clinical notes regarding patient’s condition and care given.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.
• Participates in on-call duties.
• Ensures arrangements for equipment and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Fulfills continuing education and in-service requirements.
• Active participation in quality improvement activities.
